Please report your incident using one of the following methods:
This is our standard and most secure method of receiving reports. To use the form you need to be on the Scottish wide area network (SWAN) and you need to register first by emailing us at email@example.com. When you click the submit button on the webform wou will receive an instant notification to confirm we have received your report. An automated internal message also alerts the Duty Co-ordinator.
• PDF form
If you are not on SWAN, or if you have not yet registered and need to send your report immediately, you can use the PDF form and email it to firstname.lastname@example.org. You will receive a notification to confirm we have received your report. This will normally happen within three days, but please contact us if you have not received a notification after seven days.
Please ensure any devices involved in the incident have been retained and stored safely in a secure quarantine area. Hazardous items should not be sent through the post.
A Contamination Status Certificate must be attached to all quarantined items which have (or may have) been in contact with contamination sources such as body fluids and tissues. DECON1
