Before reporting your incident, please read the important notes for patients and members of the public, or for NHS and local authority staff (and contractors). By proceeding you indicate your agreement.
Please report your incident using one of the following methods:
• On-line This is our preferred and most secure method of receiving reports
• Adverse Incident Report Form
You will normally receive an acknowledgement within three days of your report being logged. Please contact us if you have not received an acknowledgement after seven days.
Please ensure any devices involved in the incident have been retained and stored safely in a secure quarantine area. Hazardous items should not be sent through the post.
A Contamination Status Certificate must be attached to all quarantined items which have (or may have) been in contact with contamination sources such as body fluids and tissues. DECON1
