Incidents should be reported using one of the following methods. However, please note that electronic reporting is not always suitable i.e. for confidential information and it is therefore recommended that the guidance notes be referred to before using this method.

A Contamination Status Certificate must accompany all products, devices and samples which have been or could have been in contact with blood, other body fluids or pathological samples. DECON1
