Who we are
Part of NHS National Services Scotland, the Incident Reporting and Investigation Centre (IRIC) aims to improve the safety of equipment and environments in Scotland’s care services. We have a specialist remit covering medical devices, estates and facilities, and social care equipment.
We support Scotland’s health boards and local authorities on behalf of the Scottish Government. We are responsible for:
• the national adverse incident reporting system
We receive, assess and analyse adverse incident reports. We may investigate the problem depending on how serious it is. We record incidents for trending and surveillance purposes to help prevent similar occurrences in the future, even when they are not investigated.
• the national Safety alert cascade systems
We work with Health Improvement Scotland, the Medicines and Healthcare products Regulatory Agency and other partner organisations to produce and distribute safety warnings
